Application Procedures

The faculty review committee will select the strongest evidence-based case for exemplary online teaching based upon application packets submitted. The Dziuban Award Packet Submission Review Rubric will guide the committee’s selection.

Application will be via Qualtrics form where you will be asked to submit the following:

  1. Course number and section of the “W” or “V” course with a High Quality designation
  2. Instructor narrative
    • The instructor narrative should describe how your online teaching methods are exemplary. Since this is a teaching award, be sure to include evidence that highlights your methods for exemplary online teaching.
    • Additionally, the instructor narrative should reference the sources of supporting evidence/data listed below.
  3. Supporting evidence/data (can be submitted as documents or explanatory videos):
    • Student Engagement
    • Learning gains
    • Student Satisfaction

For additional information regarding sources of evidence, instructor narrative, and assembling a cohesive packet, please refer to the appropriate recorded sessions in the Dziuban Award Workshop Series.

Submission Options

Applicants are encouraged to use the Dziuban Award Packet Submission Review Rubric for a self-evaluation of their packet before submitting. Additionally, it is strongly recommended that applicants review the Dziuban Award workshop series to help prepare for an award submission.

Option 1: You may submit using the following document template to expedite your preparations:

Option 2: If it is impractical to include all evidence in a single document, please prepare and upload to the Qualtrics form one or more supporting files as appropriate for each of the following categories: student engagement, learning gains, and student satisfaction.
